Agreed on this, Tim, 100%! I have come across records from possibly well meaning clinicians that appear deskilled. Twenty SOAPs or more in a row, sometimes up to 100 visits that were all cut and paste subjective and objective. I find it hard to believe they had great SOAPs prior to adopting EMR, it's just now they look more presentable and are auto-generated.
